clearcoat failure...

I have several questions for the experts, about "clearcoat failure"
what is the cause...
what is the cure...

my situation 2001 VW Passat, paint is/has been in excellent condition...
in August I clayed the car, used WG TSR, then WG Finishing Glaze, then WG DGPS a couple days later topped with Pinnacle Souveran. then last month another coat of Souveran...
and it looks beautiful... this past weekend, I was going to add another coat of Souveran
and I found this small spot, and was told it is clearcoat failure... it is on the rear door, near the handle, on a relative flat section, there is no sign of a door contact "ding"
my first thought was it was the DGPS, but was told it was clearcoat failure...

and help or ideas will be greatly appreciated...

yup clear didnt seem to be attached to the doory properly in that spot...flaked off
