Chris, these questions fall into three categories.

Category 1 is basic info, So:

1. Call DBPR in Florida

2. Contact an insurance agent.

Category 2 is basic detailing information

#5, #7, #8, snd #9 are all questions you should be able to answer for yourself from your experience. Why don't you tell us what you've been doing or using for the past year or so and we can compare and contrast to what works for us.

Category 3 is basic business questions:

#4, #5, #10, #11, #12, #13, #14, #16, and #17 are all effectively answered in basic college level business courses, or through materials that should be available through your local chamber of commerce or community college. Give them a ring.

The remaining questions are generally personal responses so I'll address them here:

#16 I don't care for fabric upholstery or engines
#18. No
#19 Face to face
#20 I individually quote every person

Your post makes it seem like you want someone to tell you how to set yourself up in business. Thousands of books have been written on that subject. Homestly, if you haven't been doing this seriously for at least a year, it seems a bit premature to start thinking of doing it as a business.
