I order a ream of "blank" carbonless paper (2 pages), this is how I make my invoices, terms & condition form copies (white copy for me, yellow for the client) so I don't have to write twice, works great.

Just wasn't sure if the client needed more paperwork from me. I'm going to add the VIF to my carbonless forms list.

Suggestion:
Under maintenance programs, you can list 2 sets of prices. One for occasional customers and one for regular Customers. Regular customers get a better price. At the bottom you write, something along the lines of 'you are considered a regular customer after 3 washes' or something like that.
This may help encourage an occasional customer become a regular(everybody likes to drive a clean car). Kind of like the buy 9 and get the 10th free system everyone and their grandma out there uses.

I'll put together all the information and start a new post. Yes you can feed the carbonless paper through a printer, I use a Lexmark Pro715 inkjet.

I added in the top left corner of the VIF - "Invoice Ref.# 00000" as a cross reference to tie both documents together.

Yes.

I'll add the suggestions made in the last day or two and update the form.

I'm hoping people that have it will use it and post back here their feedback as we really should be able to get to a "final version".
