After all of those suspected problems that you tried I would think that it may be a mechanical issue perhaps? Maybe check the angle of the wiper against the windshield. Your description would lead me to believe it's a contact issue rather than a lubricant/grinding/wiper issue.

You use waxes around the house so that you can get that "beading and sheeting action" in places like the shower or counter tops.
