Lake Country Hydro-tech Cyan Cutting pad - disintegrated right away?

I ordered some new pads and some Sonar compound and polish to work on some trail rash on my Wrangler. I was using my Dewalt cordless 20V XR Orbital polisher, the LC Hydotech cyan cutting pad, and almost right away it started disintegrating from the center. I've never seen a pad failure like this, especially on a brand new pad. Did I do something wrong or was it a defective pad? Can't get the pic to upload.

Send your supplier a picture of it and I'll bet a new one will be on the way!
