Feedback Wanted: My version of Mike's form

Hey everyone,
Just went through the bootcamp a couple of weeks ago and want to start using Mike's form. I tweaked it a bit to include interior and add-ons. I also found a site to get images specific to the vehicle. In my case, I'll be doing a Chevy Silverado on Monday.

I wanted to try to make it a one page document (front and back).

Let me know what you think. Also, I really haven't decided on a real company name yet, so for now it's just "Andy's".
